Job Title:  Medical Social Worker

Job Summary

A Medical Social Worker will work closely with doctors, nurses, therapists (occupational, physiotherapy, speech and language), psychologists, and other allied health professionals to assess and address the social, emotional, and practical needs of patients and their families. You will act as a vital link between patients, families, and the healthcare team, helping to facilitate communication, advocate for patient and family needs, and provide holistic, patient-centered support.

Responsible for identifying psychosocial issues affecting patient care, coordinating appropriate interventions, and connecting patients and families to community resources and support systems. This includes collaborating with external agencies such as schools, social services, religious organizations, and other relevant bodies, depending on the individual needs of each cases, support discharge planning, contribute to case discussions, maintain accurate documentation, and promote overall well-being by ensuring continuity of care beyond the clinical setting.

Principle Duties and Responsibilities

• Manage assigned patient cases in accordance with social work standards and ethical guidelines, ensuring holistic support for patients and families.
• Conduct thorough assessments and interventions, including follow-up calls and home visits, to address patients' social and emotional needs.
• Collaborate effectively with multidisciplinary teams and external agencies (e.g., schools, social services, religious organizations) to explore and implement appropriate support measures.
• Act as an advocate for patients by facilitating communication and coordinating with relevant agencies through report writing and discussions.
• Facilitate clear and effective communication between patients, families, the medical team, and other stakeholders to support informed decision-making and coordinated care.
• Take initiative in assigned projects and tasks, contributing ideas to enhance the quality and effectiveness of Medical Social Work services.
• Maintain professionalism, confidentiality, and adherence to ethical standards in all duties and interactions.

Professional Requirement

1-3 years of working experience in related field

Educational Requirement

Bachelor's Degree in Social Work 

Specialised Skills/Knowledge/Attribute Requirement

• Experience in social work assessment and intervention

• Proficiency in both Malay and English

• Ability to work independently with minimal supervision

• Strong interpersonal and communication skills

• Ability to prioritize tasks and manage multiple cases effectively

• Good problem-solving and judgment skills

• Familiarity with Microsoft Office applications

• Initiative and self-motivation

• Ability to maintain patient confidentiality

• Team player with the ability to build strong working relationships
