Job Title:  Senior Medical Officer

Job Summary

The Senior Medical Officer (SMO) for neurology is a registrar‑level clinician responsible for first‑line neurology assessment and ongoing inpatient and outpatient management under consultant governance. The role includes clinical assessment, initial treatment, follow‑up of common neurological conditions, acute neurology coverage, multidisciplinary collaboration, and participation in governance and education activities, consistent with regional neurosciences centre standards. The SMO is expected to manage routine neurology presentations independently within established care pathways and to escalate complex, atypical or high risk cases promptly to senior Neurologists.

Principles Duties and Responsibilities

  • Clinical Care
    • Provide registrar‑level neurology care under consultant supervision, managing day‑to‑day patient care in assigned clinical areas.
    • Conduct comprehensive neurological assessments, develop differentials and initiate first‑line investigations and treatment plans.
    • Review clinical progress, interpret routine results, adjust management within agreed pathways, and escalate appropriately. 
    • Provide patient and family education regarding diagnosis, investigations, treatment and safety advice.

 

  • Inpatient & Ward Responsibilities
    • Lead routine inpatient reviews and participate in consultant‑led ward rounds.
    • Ensure execution of care plans, coordination of investigations, and communication with allied health teams.
    • Respond to inpatient neurology referrals with timely assessment and clear treatment plans.
    • Ensure discharge readiness through completion of investigations, clear documentation, follow‑up planning and safety‑netting.

 

  • Emergency & Acute Neurology
    • Serve as first‑line neurology cover for acute presentations including seizures, acute headaches, neuromuscular weakness and altered mental status.
    • Support activation of the acute stroke pathway, including structured neurological assessment, coordinating urgent imaging, and preparing cases for consultant decision‑making.
    • Initiate stabilisation and escalate time‑critical decisions appropriately.
    • Ensure accurate documentation and handover for all acute neurology cases.

 

  • Diagnostic & Procedural Skills
    • Perform lumbar punctures and other basic neurology procedures within credentialed scope.
    • Assist in advanced diagnostic or therapeutic procedures when required.
    • Apply working knowledge of indications for EEG and EMG/NCS, ensuring appropriate referrals, scheduling and follow‑up integration.
    • Document procedural details and monitoring requirements accurately.

 

  • Outpatient Neurology Services
    • Run supervised outpatient clinics for stable and follow‑up neurology cases as delegated.
    • Conduct pre‑assessment and case preparation for consultant clinics.
    • Support outpatient triage for referral appropriateness and urgency.

 

  • Multidisciplinary Collaboration
    • Participate in multidisciplinary meetings, case discussions and rehabilitation planning.
    • Coordinate referrals to neuroradiology, neurosurgery, rehabilitation and allied health services.

 

  • Governance, Quality & Education
    • Participate in clinical audits, quality improvement initiatives and patient safety activities.
    • Maintain high standards of documentation, handover and communication.
    • Ensure high quality documentation, timely completion of clinical records, and adherence to neurology service protocols and clinical pathways, including support for achievement of service performance indicators.
    • Supervise and guide Medical Officers and House Officers, providing teaching and feedback.
    • Contribute to departmental training and service development initiatives.

 

  • Research & Service Development
    • Contribute to departmental registries, research activities and service development projects as required

Professional Requirements

  • Minimum five (5) to seven (7) years post‑qualification clinical practice.
  • Experience managing common neurology presentations including stroke/TIA, seizures, headache disorders, neuropathies and demyelinating disorders.
  • Competence in lumbar punctures and acute neurology referrals is desirable.

Educational Requirements

Primary Medical Degree (MBBS or equivalent) recognised by the Brunei Medical Board (BMB).

 

Candidate must have one of the following:

  • Completion of Basic Neurology Specialty Training (NST)
    • Completed Basic Neurology Specialty Training with Internal Medicine experience within a recognised neurology training programme.
    • Passed the Basic NST interval examination (or equivalent competency assessment).
    • At least one (1) to two (2) years of supervised experience working within Internal Medicine specialties and involvement in acute medical take.

or

  • MRCP / MRCPI with Neurology Experience
    • Completed MRCP(UK), MRCPI or another BMB‑recognised equivalent postgraduate qualification; and
    • At least two (2) to three (3) years of supervised neurology experience, including rotations or registrar‑level service.

 

Specialised Skills/Knowledge/Attribute Requirement

  • Clinical Competencies
    • Strong neurological assessment skills, recognising red flags and escalating appropriately.
    • Ability to interpret CT/MRI relevant to neurology and identify indications for EEG/EMG.
    • Competence in acute neurology pathways including stroke and seizure management.
