Job Title: Specialist (Medical Oncology)

Job Summary
- Provide high-level clinical expertise in managing patients referred to Palliative Care across inpatient, outpatient, and community settings.
- Lead the development and provision of specialist palliative services, ensuring a person-centered approach and seamless continuity between hospital and home.
- Champion clinical governance, service development, and the training of medical personnel.
- Shape departmental policies to ensure operational efficiency and continuous service improvement.
Principles Duties and Responsibilities
- Clinical Case Management
- Lead multidisciplinary teams (MDT) to provide comprehensive care for both oncology and non-oncology palliative cases.
- Regularly evaluate and adjust management plans in response to changes in patient condition.
- Offer thorough education regarding disease progression and treatment options to patients and their families.
- Independently initiate and execute evidence-based palliative treatment plans.
- Patient Referrals & Ward Management
- Oversee transitions of care between specialists and support services to ensure ongoing needs are met.
- Perform regular clinical assessments and ensure meticulous documentation of changes in patient status.
- Collaborate with nursing teams to ensure prescribed palliative treatments are correctly implemented.
- Palliative Patient Assessment
- Perform detailed consultations for new patients, ensuring thorough histories and diagnostic reviews are completed.
- Maintain accurate medical records, documenting all clinical findings and treatment plans promptly.
- Ensure timely access to diagnostic tests to facilitate rapid initiation of symptom management.
- Enhanced Community Care Responsibilities
- 24/7 Home Support Integration: Lead the provision of indirect care by supporting community-based healthcare professionals to ensure seamless continuity of care between the hospital and the home.
- Mobile Symptom Management: Oversee the transition of care between hospital specialists and community support services, ensuring patients' ongoing symptom management needs are met without requiring hospital admission.
- GP & Primary Care Collaboration: Provide advice and support extended team of GPs and community nurses, providing expert guidance on complex palliative cases in the community.
- Caregiver Empowerment: Provide thorough education to family members regarding disease progression and home-based management strategies to prevent caregiver burnout and social readmissions.
- Strategic Alignment (BRUMAP): Operationalize palliative component in Brunei Darussalam National Multisectoral Action Plan for the Prevention and Control of Noncommunicable Diseases (BRUMAP) to shift the focus of clinical practice toward home-centric care excellence.
Professional Requirements
- A minimum of seven (7) to ten (10) years of full-time clinical training and working experience post registration in a government or other private health facilities is essential.
Educational Requirements
- Possess a Primary Medical Degree, MBBS or equivalent.
- Postgraduate training and qualification in Paliative Care is required.
- Professional Fellowship (e.g., FRS or equivalent) with specialized training in Palliative Care.
Specialised Skills/Knowledge/Attribute Requirement
- Clinical Expertise: In-depth knowledge of palliative protocols, including complex symptom management and end-of-life care.
- Holistic Approach: Proficiency in addressing physical, psychological, social, and spiritual needs while facilitating family involvement.
- Ethics & Autonomy: Strong understanding of ethical considerations, informed consent, and patient autonomy in palliative settings.
- MDT Leadership: Ability to effectively coordinate with nursing, medical, and allied health professionals in complex cases.
